How do I become a patient?
If you would like to become one of our patients, please take your Medical Card (if available) to your local health centre, where you will be asked to complete an application form and a new patient questionnaire. You will also be offered the chance to register for online services as part of the registration process.
When you register you will be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Once registered all patients are allocated a named GP. If you would like to find out who your named GP is then please speak to reception.
If you have a medical or follow up need at the time of your application it may be worth booking an appointment to see either a doctor or nurse relevant to that need at that time.
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice. You can register as a temporary patient for up to three months.
Although you will be temporarily registered with us you will still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
Out of Area
All of our practice's have their own practice area and we are only able to accept new patients from within these areas. If you move outside these boundaries we may ask you to register with another practice.
