Sick (fit) Note

The Statement of Fitness for Work, commonly known as the ‘fit note’ or Med 3 form can be requested through our online portal or via telephone call to the practice but only once you have been off sick for more than one week

If you are off work for 7 or fewer days you are able to complete a self-declaration for sickness absence.

You do not need to pay for a fit note if you’re off sick for more than 7 days.

If your employer asks for evidence for the first 7 days of sickness, a healthcare professional may charge a fee for this. Your employer should pay the fee.

Back dating

It is possible for a sick note to be backdated so you do not need to be seen urgently when one expires. A sick note can be back dated at the doctor’s discretion. A doctor may also extend a sick note without seeing you again.

A sick note cannot be future dated.

Sick note after a hospital stay.

The hospital should issue sick notes for the period of a hospital stay and expected recovery time, this should be provided at discharge.
